Pool Addiction Counselor-BHU-(Days)
Interviews patients/families/significant others within one working day of referral to assess relevant psychosocial, medical, economical, emotional, cultural, and religious/issue, actual or potential, that would impact patient’s health care needs. Explains the social work role. Completes assessment and prioritize high-risk patients/families as defined by policy. Documents on patient medical record when services are provided, including assessment, interventions, education, family conferences, interdisciplinary plan of care, advance directives and patients progress toward discharge plan in the time frames defined by policy.
Must be a graduate of an accredited school with a Masters Degree in Professional mental health counseling. Prefer two years of Case Management/Social Services experience in a healthcare related field. Acute care hospital experience preferred. A current LA. License is required. Must possess excellent organizational and time management skills. Ability to set priorities and to work independently. Strong oral and written communication skills required to interact with patients, families, physicians and co-workers.
